As primary care providers, it is our responsibility to manage your overall health and well-being. We will work with other health-care providers - including hospitals, specialists, and various outpatient services to provide you with complete, coordinated care. You will also be connected with health coaches, support groups, and other services designed to get you healthy and we will remind you when it is time for your regular check-up and health screenings to help you stay healthy.


We encourage all patients to participate in preventative health care with annual physicals, immunizations, and screenings as this has been shown to identify risk factors and prevent disease.


 We recommend contacting your insurance company to find out what preventative care options are available to you at little to no cost.

What is a provider? 

  • A provider is a healthcare professional such as a doctor, nurse practitioner, physician's assistant, or other professional trained to take care of your specific health care needs.

What is primary care?

  • Primary care is total body, comprehensive care which includes health promotion, disease prevention, health maintenance, diagnosis and treatment of acute and chronic illness (AAFP, 2016).
